- I (We) have operated a bicycle before and know how to operate the type of bicycle rented.
- I (We) have been instructed by Corliss Bike and Supply on the operation of the bicycle and helmet and fully understand how to operate them.
- I (We) take total responsibility for my (our) own safety and assume the risk of any injury that may result from the use of bicycles rented from Corliss Bike and Supply.
- Before riding or using the bicycle I (we) state and acknowledge that I am (we are) not intoxicated (includes any drugs, alcohol or prescription medications) and do not have any physical disorders that would interfere with the safe operation of the bicycle at any time during the rental period.
- I (We) have inspected the rental bicycle(s) and agree that the equipment is in good working condition. I (We) accept the bicycle(s) as is and agree to be responsible for the care of same while in my (our) possession.
- I (We) agree to wear a protective helmet at all times while operating the bicycle(s). I (We) have accepted the protective helmet that has been provided by Corliss Bike and Supply free of charge as part of this bicycle(s) rental agreement.
- I am (We are) responsible for the replacement at full retail value of any bicycle(s) rented under this agreement that is not returned or is damaged during the rental period. I (We) agree that if the bicycle(s) are not returned on the due date, I (we) will be responsible for a daily charge for every day the bicycle is late.
- I am (We are) responsible for any damage to privately owner property or personal property while operating the bicycle(s).
- I (We) fully and completely RELEASE, ACQUIT and FOREVER DISCHARGE Corliss Bike and Supply, its owners, managers, staff or suppliers from any and all claims, demand and causes of action of any nature whatsoever, including without limitation, claims of negligence arising out of or relating to any injuries of any nature I (We) might sustain as a result of riding on, falling from, or being injured by the bicycle(s). I (We) waive any rights to sue or assert any such claim to Corliss Bike and Supply its owners, managers, staff or suppliers.
